Public Housing & Section 8 Housing Choice Voucher Application Information & Instructions


Required Documents for Application Submission:

You must provide the following:

  • Social Security Card for all household members.
  • Photo ID (Driver's license or state-issued ID) for anyone 18 years of age or older.
  • Certified Birth Certificates for all household members.
  • Proof of Custody for minors (e.g., birth certificate, divorce decree, notarized statement).
  • Contact information for any non-residing parent of listed minors.
  • Any additional forms or verification as requested.

Note: Originals can be copied at the PHA office. If mailing, send copies only - not originals.


Household Members

You must list the names of all people (adults and children) who will actually be living with you, whether or not they are related to you.

All household members age 18+ will undergo a background check.


Income (for All Household Members)

You must include proof of all income sources:

  • All earnings: wages, tips, bonuses, part-time/second jobs: 6 most recent pay statements.
  • Unemployment Benefits: Unemployment Statement.
  • Family Support: Zero, Low-, or Undocumented-Income Worksheet. (available at the office)
  • Benefits: Social Security, pensions, welfare, alimony, child support: most current award letter.
  • Child Support: If children are in the home, child support verification is required.
  • SNAP, TEA, TANF: DHS documentation required.
  • Income from assets: bank interest, stock dividends.
  • Any anticipated income: (e.g., expected raise or bonus)

Note: You must have carned or unearned income in your household.

If these documents are not included with the application, it will be considered incomplete and returned to you.


Assets - Must Be Reported

You must report all assets for yourself and any adult household members:

  • Bank accounts, savings bonds, certificates of deposit. (CDs)
  • Stocks, real estate, or property.
  • Any businesses or assets sold in the last 2 years for less than full market value.
  • Example: If you sold your home to a family member for less than it was worth.

Application Requirements

  • The application must be fully completed, signed by all adult household members, and include all required documents.
  • Applications must be completed to be accepted by the PHA for processing.
  • All forms required for the PHA to process your application must be returned within 14 calendar days of the application date.
  • It documents are missing or incomplete, the application will be denied.
  • If denied, you will receive a letter at the address listed on your application.
  • Your application may be removed if you do not respond to an offer or ignore a purge/update letter.

Important: If any information is found to be false, omitted, or misleading, the application will be denied.


Waiting List Information

  • Eligible applicants go on a community-wide waiting list.
  • If and when approved, you will be added to the list by the date/time your application and required documents are received.
  • Assistance is offered based on HUD-approved policies, without discrimination.

While on the Waiting List

You must report any changes in:

  • Household members.
  • Income or assets.
  • Address or phone number.

Failure to update contact information may result in removal from the waiting list.


Penalties for Committing Fraud

If you knowingly provide false or incomplete information, you may be:

  • Evicted from your home.
  • Required to repay overpaid rental assistance.
  • Fined up to $10,000.
  • Imprisoned for up to 10 years.
  • Barred from future housing assistance.

State and local governments may impose additional penalties.


Signing the Application

Before signing:

  • Read every form completely.
  • Make sure all information is accurate and complete.
  • Do not sign if you don't understand something - ask questions first.

By signing, you are:

  • Confirming that the information is true to the best of your knowledge.
  • Subject to prosecution for fraud if you provide false or misleading information.

Important: Your application will be verified by the housing agency. HUD may also use computer matching with other agencies (Federal, State, or private) to confirm income and asset details.
